Archive for September, 2010Dirty Dozen: Lack of KnowledgeThis month’s Dirty Dozen article is about lack of knowledge and how it causes errors and accidents. When I was in China, I read a quote by Confucius: The essence of knowledge is, having it, to apply it; not having it, to confess your ignorance. It’s important to always be learning to reduce our ignorance and it is also important to keep using what we know and reviewing so that one does not forget. The article for this month contains some examples of incidents I remembered from my work in Operational Excellence. I hope you and your company perform incident analyses and develop lessons learned from accidents and near misses.
