Dirty Dozen Causes of Errors- Stress

Gordon Dupont, with System Safety Services, worked with others at TransCanada to identify causes of human error in aviation maintenance.  Those “Dirty Dozen” are applicable to other industries, including the pipeline industry.  For a few years, my partners and I have developed a Dirty Dozen calendar, along with training programs and presentations.
